- Copy of the minutes where nominee is endorsed by the P&C Association
- Statement of support
- Any other supporting files e.g. photos, a short video or other documents
Note: If you are nominating on behalf of various P&C Associations, please submit one form per P&C Association.
What happens after you submit your nomination?
- You will receive a confirmation email
- The entry will be evaluated by a panel of independent judges
- Finalists will be notified prior to the award presentation
