  • Contact details of nominee(s)
  • Copy of the minutes where nominee is endorsed by the P&C Association
  • Statement of support
  • Any other supporting files e.g. photos, a short video or other documents

Note: Please submit one form per P&C Association.

What happens after you submit your nomination?

  1. You will receive a confirmation email
  2. The entry will be evaluated by a panel of independent judges
  3. Finalists will be notified prior to the award presentation

To be able to nominate, your P&C Association must be a current member (affiliate) of P&C Federation
All nominees must be endorsed by the P&C Association and that endorsement must be recorded in the minutes at a meeting of the P&C Association.
